Roll-off release waiver

In consideration for Burgmeier's Hauling, Inc. ("Burgmeier's”) renting a container to me, the undersigned, for the disposal of municipal and/or other non-hazardous waste, on a personal use or commercial use basis, the undersigned hereby releases and forever discharges Burgmeier's and it officers, directors, employees and agents from any and all causes of actions, suites, debts, damages, claims and demands whatsoever in law or in equity, which may arise as a result of the placement, delivery, set-off, use, or removal of the container by Burgmeier's.

If the undersigned is a contractor, the contractor assumes full and complete liability on behalf of their client as per the terms and effects identified above authorizing Burgmeier’s to deliver and remove said container. Also in compliance with safety guidelines set forth by Burgmeier's Insurance Carrier, The Department Of Environmental Protection ("DEP") and The Department Of Transportation (“DOT"), the undersigned shall not load container above the MAXIMUM FILL LINE, which is designed at the top of the box. This falls under the Municipal Waste Code #285.214 B1 that states "The load shall be no higher that the solid sides of the vehicle".

By signing the General Release Form, I certify that I have read the terms hereof with a full understanding as to their terms and effects and hereby authorize Burgmeier’s to deliver and remove, to me, the undersigned for the disposal of municipal and/or other non-hazardous waste.
